Our Mission

If you attended Holy Name Seminary or Beloit Catholic High School, you may request a transcript from the Office of Catholic Schools. Complete the Transcript Request form and return it to the Office of Catholic Schools.

If you attended Queen of the Apostles High School,  you may request a transcript from the Pallottine Fathers at the following address:
